About Homeless Healthcare
We are a multi-disciplinary primary care team providing care to homeless people in Southampton.
What we do
The Homeless Healthcare Team provides primary healthcare to registered patients.
The team will register anyone who is homeless and over the age of 18* in Southampton and is not appropriately registered with a local GP.
*Homeless includes people sleeping rough, living in hostels or other temporary accommodation or refuge houses for victims of domestic violence or human trafficking, and Gypsies and Travellers on illegal encampments.
We also provides specialist mental health care to people who are homeless and who may be registered with a local GP surgery.
Accessing the service
Individuals can self-refer by presenting to the Two Saints Day Centre, 30 Cranbury Avenue in Southampton between 9am and 4pm Monday to Friday. Alternatively, you can telephone us on 0300 123 3991 to book a registration appointment.
